An Extended Family member can help you to formulate the right questions for your healthcare provider so that you will get the best care possible. We will sit in on Doctors or visiting nurse visits to listen and to help you and your family understand what the doctor or health care provider has said. We are there to remind you & answer your questions long after your visit has ended. We can be there if you are an outpatient or in a hospital or facility.

  • Senior Friendly visits. Extended Family will visit with your loved one to give the TLC that a family would if circumstances allowed them to.

  • Long Distance Care Management:
    We will help to alleviate the stress of trying to supervise your aging parents or loved one when you live hours away. Extended Family will be your long distance care consultant & advocate. Allowing them to remain in their home.

    GOING ON VACATION?

    Care Monitoring Short or Long Term:
    When you only need us for a short time or if you need a longer respite, we can help. If your loved one is at home we can look in on them while you go on vacation. We can assist you with arranging Respite care. If your loved one is in a nursing facility, we can visit with them to be sure they are getting the care that they deserve.

    NEED SOMEONE NEARBY IN CASE OF EMERGENCY?

    Emergency contact:
    Let extended Family be the person nearby for your loved one to call in times of need or for reassurance. Extended Family will go to the emergency department to offer reassurance & comfort to your loved one. We will act as the liaison for the family explaining all events to them; to help the family make informed decisions. Extended Family will stay with the senior until you arrive or until a diagnosis and a plan of treatment is decided on.

  • Guidance With The Selection Of Alternate Living Arrangements:
    We can help you to evaluate the need for Alternate Living arrangements. We can assist you to find the facility that is the right fit for your loved one. When you aren't sure what type of help your loved one needs. We will do an in home evaluation and make recommendations.

  • Resource Referrals:
    When you don't know where to turn. If we don't have the services that you need we will help you get them. Extended Family networks with many local caregiver & nursing agencies as well as with many area service providers. We can help you with almost any problem that you may encounter.

  • Assistance with finding the right medical care & equipment to allow the senior to remain comfortably at home.

  • Care Giver Support:
    Extended Family is a member of the National Family Caregivers Association; we have tips and advice to help deal with the stresses of being a caregiver. We are the ones to call when you are feeling on the edge and out of control. We are just a phone call away. We have the resources to act as an advocate for you or your family if you are unable to.

  • Correspondence With The Family:
    We will send a written report to family members at your request.

  • In home consultation:
    Our in home consultation allows us to spend time with the senior in their environment; allowing us to observe safety issues and the needs of the senior and caregiver and to make recommendations that will best benefit all involved.

  • Your privacy is important to us. We will not discuss your personal matters with family members or other care providers without your permission.
